The hospital has also agreed with recommendations to improve the handling of cases of pregnancy-induced hypertension, or elevated blood pressure, following the death of 32-year-old Corrina Medway.

Mrs Medway suffered a fatal stroke after giving birth to twins at Calvary Hospital in May 2011.

Her blood pressure readings had escalated significantly, and eventually reached catastrophic levels following the delivery of the twins.

Fast-acting medication was not used to reduce her blood pressure, and she suffered a "massive" cerebral haemorrhage.

Mrs Medway was urgently transferred to Canberra Hospital following her stroke, but never recovered.

Her death was examined extensively during an inquest last year, and Coroner Margaret Hunter delivered her findings in mid-December, strongly criticising Mrs Medway's obstetrician, Dr Andrew Foote.

Ms Hunter found Dr Foote knew of his patient's high blood pressure readings, yet failed to administer anti-hypertensive medications.

"Dr Foote's failure to treat Mrs Medway's acute pregnancy induced hypertension ... resulted in her blood pressure continuing to escalate to a critical level, which ultimately caused her cerebral haemorrhage and death," Ms Hunter found.

"I am satisfied that treatment was available within a reasonable period of time sufficient to have ameliorated the significantly high blood pressure."

The coroner made several significant recommendations to reduce the risk of such deaths in the future.

She recommended nurses, midwives, GPs, and specialists undergo specific training on pregnancy-induced hypertension.

The coroner said literature on pre-eclampsia, a pregnancy disorder characterised by high blood pressure, should be provided to pregnant women to raise awareness.

Ms Hunter also called for patient notes to be sent with women when they are discharged from the birthing suite onto the ward, and reiterated that taking contemporaneous notes was vital in significant events.

A spokesman for Calvary Hospital said on Wednesday that the hospital accepted the findings and recommendations of the coroner.

"Calvary Hospital has read the coroner's findings into this tragic incident and agree with the coroner's findings and recommendations," the spokesman said.

"Calvary takes this opportunity to again extend our deepest sympathies to the Medway family"

Dr Foote was cleared in a separate professional standards hearing into the death, which found the allegation that he failed to correctly diagnose the hypertensive disorder was not proven.

In his evidence to the ACT Coroner's Court inquest, he said he had not been told of crucial blood pressure readings, and was wary of giving anti-hypertensive drugs without a proper diagnosis.

"I think it was a very confusing picture and we were all concerned about giving anti-hypertensives if it was the wrong diagnosis," he said during the inquest.

Anti-hypertensive medication had been used successfully on Mrs Medway in the week before her death, and one expert, Professor Shaun Brennecke, told the coroner there would have been no disadvantage in administering the drugs.

She also had a family history of hypertension on her mother's side.

The obstetrician issued a statement attacking the coroner's findings days after they were delivered, saying they were based on fundamental errors of fact.

"The coroner's report was based on the assumption that Dr Foote knew of escalating blood pressure readings, however there was NO robust evidence presented at the inquest to substantiate this," a statement issued by his clinic said.

Dr Foote's statement also said two other doctors saw Mrs Medway two hours before her death and agreed with his own likely diagnosis of a pulmonary embolism.

Another physician was also said to have agreed with Dr Foote that anti-hypertensive medication should not be used in cases of pulmonary embolism.

Dr Foote said he had been cleared by the Australian Health Practitioner Regulation Agency.
