Stefan Trpcevski, also known as Bitola Marmalado, faces 118 criminal offences after authorities discovered 27 dogs in poor health and shocking conditions at homes in Macquarie and Ainslie in late 2013.

His mother Lidija Ski and brother Aleksander Trpcevski are also before the ACT Magistrates Court.

No formal pleas have been entered.

Government inspectors found 21 dogs chained up in the Macquarie backyard that was littered with cars, junk, television, tyres, metal sheeting and other debris, court documents alleged.

Another six dogs were found at the Ainslie property, allegedly in varying degrees of starvation.

The trio faces more than 200 charges including failure to seek veterinary treatment and failure to provide an animal with food and water.

They were also accused of keeping unregistered dogs that had not been desexed, and failing to exercise a confined animal.

Stefan Trpcevski and Ms Ski together faced more than 40 fresh allegations of animal cruelty when they appeared before Chief Magistrate Lorraine Walker on Monday.

Prosecutor Katrina McKenzie said the pair had yet to be formally charged.

The court heard Aleksander Trpcevski, who faces 30 charges, was not in court as he was receiving treatment for mental illness.

Ms Walker adjourned the matter so the new charges could be considered.

She ordered the accused's defence lawyers to indicate whether their clients would plead guilty or not guilty to each of the offences before the next court appearance.

Ms Walker also requested submissions on Aleksander Trpcevski's mental health, and said he should attend court next month unless he was medically unable.

The matter will be back in court on May 20.
