The 61-year-old and 48-year-old were killed in a head-on collision on the Midland Highway in Avonmore, between Goornong and Elmore.
Police said they were travelling home when their Nissan Pulsar collided with a silver Holden around 1.15pm.
Officers confirmed one vehicle veered on to the wrong side of the road.
A 22-year-old woman and a 26-year-old man from Ballarat were airlifted to the Alfred Hospital in Melbourne in a stable condition.
Neither vehicle was travelling above the 100 kilometre per hour speed limit and investigators suspect either fatigue or distraction caused the crash.
Major Collision Investigation Unit detective sergeant Darren Williams made an impassioned plea for motorists to stay safe on the road.
