Police were called to Kalamunda Road in Maida Vale at about 3am after the 26-year-old woman, who suffered numerous cuts and lacerations, got out of a gold four-wheel drive she was a passenger in.
It is believed the woman, from Orelia, was attacked by her former partner with an "edged weapon" as they drove from Rivervale.
The woman was helped by a witness as the car she was travelling in drove off.
Police wish to speak to 32-year-old Shane Dowden who is driving the gold Ford Explorer four-wheel-drive with NSW registeration BQ0 7BW and also the driver of a black taxi that was at Kalamunda Road who may have witnessed the incident.
Mr Dowden may be armed and should not be approached.
