Small blitzed her rivals in Samoa, finishing four seconds ahead of the next best athlete in the women's 800 metres.
It adds to her Youth Olympic gold at the end of last year and the valuable experience she gained at the Commonwealth Games on the Gold Coast.
Small, 18, will be hoping to make her senior Olympic debut in Tokyo next year.
Her result in Apia continues a good run of form for Canberra athletes after Kelsey-Lee Barber threw an Olympic qualifier in the javelin last month.
