Worksafe ACT director Greg Jones on Wednesday confirmed the regulator was still investigating the incident, and had not yet imposed any penalties on the companies involved.
While the regulator could yet take some action, a safety notice sent out on Wednesday confirmed the regulator putting the blame down to poor communication by the crane crew regarding the weight of a load placed on the bond deck.
It said the load exceeded the four tonne limit of the trestles, was put in the wrong place and timbers used to spread the load were "inadequate and too short".
