Emergency services were called to the Snowy Mountains Highway, 20 kilometres south-east of Talbingo at about 2pm, after reports of a crash involving a truck and a car.
A Toyota Corolla travelling north and a prime mover travelling south reportedly collided head-on in the northbound lanes, before the truck went over an embankment.
The Sydney driver of the car died at the scene.
The woman's only passenger, an 11-year-old boy, was trapped for a short time before being freed.
He was treated by NSW Ambulance paramedics for arm injuries before being flown to hospital where he's believed to be in a stable condition.
The male driver of the heavy vehicle, also aged 48, was taken to hospital to be checked and undergo mandatory testing.
The section of the Snowy Mountains Highway which had been closed, between Miles Franklin Drive at Talbingo and Link Road, has since been reopened.
