A car was travelling outbound on the freeway near Williamstown Road at Spotswood when it collided with the truck just after 10.30am on Tuesday, police say.
Fire crews arrived to find a truck had rolled over and worked to contain smoke coming from the vehicles, Fire Rescue Victoria says.
The male driver of the car and a male passenger, both believed to be in their 20s, were taken to Royal Melbourne Hospital in a critical condition.
One of the men was trapped for some time, Ambulance Victoria says.
Three other men in the car and two men in the truck were assessed by paramedics.
A person in their 20s and another in their 50s have minor injuries. Both were taken to Footscray Hospital in a stable condition.
Police confirmed three people were arrested and have since been released.
Callers to Melbourne radio 3AW reported a silver sedan crossing four lanes of the bridge at speed.
"It was like a bullet," one caller said.
