A man in the other car was trapped in the wreckage after the smash at Coolangatta, on the Shoalhaven River on Thursday afternoon.
Paramedics couldn't revive the elderly woman, while the 41-year-old man was eventually pulled from the crumpled car and airlifted to St George Hospital with neck, chest and leg injuries.
He remains in hospital in a stable condition.
