The ACT held a seven-goal advantage at half-time, but a monster third quarter from NSW helped it to a 45-43 victory in their group match on the first day of the titles in Adelaide on Tuesday.
NSW is one of the favourites for the title having won five of the past six under-21 Australian championships.
However, it was given a huge fright as the ACT took a seven-goal lead into half-time.
Centre Bec Collis and wing attack Georgia Clayden were outstanding as the ACT forced turnovers and capitalised in its attacking third.
NSW came out of the sheds with extra intensity, winning the third quarter by 10 goals to lead by three going into the final stanza.
The ACT wouldn't go away and was up by one goal with five minutes to go, but NSW showed why it is the best in the country to scrap out the victory.
ACT goal shooter Georgia Marshall enjoyed an accurate game with 33 goals from 40 attempts at 83 per cent.
The ACT will search for its first victory of the tournament when it takes on South Australia (10.30am) and Western Australia (4pm) on Wednesday.
