Police say the cabbie picked up a 30-year-old man from a home in Sydney's west on Wednesday morning, before taking him to a home at an address in Riverstone.
Both were shot while still in the car outside the home, with the passenger - a member of an outlaw motorcycle gang - being hit multiple times.
The taxi driver then drove to Hawkesbury Hospital, where both men were treated for gunshot wounds and remain in a serious condition.
Detective Superintendent Robert Critchlow told reporters the passenger was well known to police.
"We know him fairly well," he said.
Mr Critchlow said it was unacceptable a bystander was harmed in the attack.
"You fire enough shots at a person you want to kill and you hit someone innocent, that's really unacceptable," he said.
"People in Sydney should be able to live their lives without being at risk from a stray bullet from one fool shooting another fool."
