The big-serving Isner won in straight sets but it was a tight final, the 36-year-old beating his 19-year-old opponent 7-6 (10-8) 7-5.
For world No.115 Nakashima it was another frustrating Sunday, after losing the final at Los Cabos last weekend to Britain's Cameron Norrie.
For the 35th-ranked Isner, his 16th career title brought sweet revenge after he lost to Nakashima in the Los Cabos semi-finals.
As could be expected from the scoreline, the stats reflected a close encounter. Isner saved three of three break points against him, Nakashima saved four of five.
Isner won 82 points to Nakashima's 78, but found an edge in pounding down 21 aces to Nakashima's 12.
Isner becomes only the fourth current player to have won a tour-level title six times or more.
Roger Federer achieved the feat in seven different tournaments while Rafael Nadal and Novak Djokovic have done it four times apiece.
In the men's doubles, Australia's Jordan Thompson and American partner Steve Johnston were beaten in the final by Reilly Opelka and Jannik Sinner 6-4 6-7 (8-6) 10-3.
