Hussein Al Mansoory, aged 12, was found at a medical centre in Auburn about 12.40pm on March 11 after being missing for three days.
It came just hours after Auburn police superintendent Simon Glasser asked locals to check possible hiding spots in buildings.
"One of the staff members thought to check the stairwell which is exactly what we were asking," he said.
"He was found well; he smiled, he was sitting up and he's been taken to hospital for a check-up.
"His family were told immediately that we'd found him and they were ecstatic and attended the scene straight away."
Hussein went missing from his home on March 9 and was last seen at 10.30am running from Auburn Memorial Park towards the intersection of Station Rd and Rawson St.
He is non-verbal and lives with down syndrome and autism, and was last seen wearing a white shirt and a nappy; he wasn't wearing shoes.
Earlier, superintendent Glasser had informed reporters the search was "at a critical point".
"We're highly concerned, this is as high-risk as it gets for us as a missing person," he told reporters on March 11.
"Our last confirmed sighting was when he went missing on Saturday."
Superintendent Glasser said Hussein's family were "quite distraught" and assisting in the search.
He asked people in the Auburn area with large premises such as a unit block or business to check for Hussein in places accessible to the public such as fire escapes.
Police believe Hussein may have been in the area around Norval St and Hevington Rd, Auburn, about 9pm on March 9.
More than 100 SES volunteers were involved in the search on March 11, alongside police, airwing and other specialist units.
