The private hospital could introduce paid or timed parking in the new year to try to ease parking shortages at the busy Deakin site.
The John James Foundation has unveiled a master plan to expand the hospital and update facilities that were built in the 1960s.
The plan will require hundreds of millions in capital works and could take from 15 to 20 years to complete.
The hospital is owned by the John James Memorial Foundation and is operated by Little Company of Mary Healthcare.
The foundation said in its annual report that the declining value of its existing properties made the works "imperative". An upgrade of the hospital's maternity facilities will begin over Christmas.
Foundation chief executive Phil Greenwood said the maternity facilities would be followed next year by building works for one or two new operating theatres.
The foundation planned to expand on its existing rehabilitation and orthopaedic services, he said, and hoped to introduce other clinical services it did not now offer.
He said the timing and scale of the works would depend on financing.
"It will be a program of building over a period of time," Mr Greenwood said. "We need to have a building program that doesn't impact on the existing hospital. We have to plan it carefully so we can build facilities that we can decant existing services into."
The hospital now has 155 beds but wants to increase this to 260.
"Additional ward space is the big push," Mr Greenwood said. "Increasing the number of beds available enhances the throughput of the treatments."
Mr Greenwood said that while creating more operating space was the most immediate issue, the hospital also had to find a solution to its parking woes next year. Demand for parking at the site is outstripping the supply, with hundreds of staff, patients and visitors sharing the parking facilities each day.
Mr Greenwood said the hospital would probably introduce timed or paid parking space within the next six months.
"There has to be controls - paid or timed parking, or both," he said. "We're certainly not interested in parking for the sake of revenue raising. We're trying to keep this as a private hospital that is a quality facility and suitable for the people that need to access it."
Mr Greenwood said the foundation did not know how much it would charge for parking if it decided to impose a fee, but the revenue would go back into the hospital or towards the foundation's charity work.
The foundation's gross revenue for the 2012-13 financial year was $4.8 million and its operating expenses were $2.55 million.
